REQUIREMENTS FOR REPORT OF MARRIAGE

For Marriage Contracts issued in AUSTRIA:

1. Report of Marriage form - To be accomplished in four (4) original copies;
2. Marriage Certificate of the applicant – Original;
   1. International Version issued by the Standesamt in Austria
      • Formblatt B – Auszug aus dem Heiratseintrag
   2. Three (3) photocopies of front and back page;
3. Passports of husband and wife - Original
   1. For Filipino spouse (data page and last two pages) - Three (3) photocopies
   2. For Foreign spouse (data page only) - Three (3) photocopies
4. Four (4) photographs of wife
5. Four (4) photographs of husband
6. Fee: € 25.00

NOTES:

In Formblatt B – Auszug aus dem Heiratseintrag, the MIDDLE NAME of the Filipino spouse BEFORE marriage must be reflected.

In filling-up the report of marriage form, under the wife’s column, please indicate the MIDDLE NAME and LAST NAME of the Filipino spouse BEFORE marriage.

In the space provided for “Name & Signature of Person Reporting the Marriage” the FULL MARRIED NAME of the Filipino spouse must be written.

ePASSPORT APPLICATION REFLECTING MARRIED NAME

1. Personal appearance of the Applicant;
2. Accomplished ePassport Application Form;
3. One (1) Photo: Size – 2.5cm x 2.5 cm, – White Background
4. Marriage Certificate of the applicant – Original 
   1. International Version issued by the Standesamt in Austria
      • Formblatt B – Auszug aus dem Heiratseintrag
   2. One (1) Photocopy of front and back page
5. Birth Certificate of the Filipino spouse – Original and a photocopy;
6. Passports of husband and wife
   1. For Filipino spouse (data page and last two pages) – One (1) Photocopy
   2. For Foreign spouse (data page only) – One (1) Photocopy
7. Fee: € 60.00

1. Report of Marriage form - To be accomplished in four (4) original copies;
2. Marriage Certificate of the applicant – Original;
   1. International Version and authenticated by the respective Ministry of Foreign Affairs (Croatia, Slovakia and Slovenia);
   2. Three (3) photocopies of all the pages;
3. Passports of husband and wife - Original
   1. For Filipino spouse (data page and last two pages) - Three (3) photocopies
   2. For Foreign spouse (data page only) -  - Three (3) photocopies
4. Four (4) photographs of wife
5. Four (4) photographs of husband
6. Fee: € 25.00
