REQUIREMENTS FOR REPORT OF DEATH

For Death Certificates issued in Austria:

1. Report of Death form to be accomplished in four (4) original copies;
2. Original Death Certificate (TODESBESTÄTIGUNG/STERBEBUCH) (international Version or with English translation) of deceased with four

(4) photocopies;

1. Original and photocopy of deceased's passport;
2. Original and photocopy of deceased's birth certificate, if available;
3. If married, original and photocopy of deceased's marriage contract, if available;
4. Fee: € 25.00

For Death Certificates issued in CROATIA, SLOVAKIA and SLOVENIA:

1. Report of Death form (to be accomplished in four (4) original copies);
2. Original Death Certificate authenticated by the respective Foreign Ministry with four (4) photocopies;
3. English translation of the authenticated birth certificate with four (4) photocopies;
4. Original and photocopy of deceased's passport;
5. Original and photocopy of deceased's birth certificate;
6. If married, original and photocopy of deceased's marriage contract;
7. Fee: € 25.00
