REQUIREMENTS FOR REPORT OF BIRTH
For Birth Certificates issued in AUSTRIA:
- Report of Birth form - To be accomplished in four (4) original copies;
- Birth Certificate of the applicant - Original
- International Version issued by the Standesamt in Austria. (Formblatt A) - Auszug aus dem Geburtseintrag
- Three (3) photocopies of front and back page;
- Marriage Contract or Report of Marriage of the parents of the applicant, if applicable - Original and three (3) photocopies;
- Filipino Parent's Birth Certificate - Original and Photocopy;
- Parent's latest passports - Original;
- Three (3) photocopies of mother's passport (data page and last page)
- Three (3) photocopies of father's passport (data page and last page)
- Fee: € 25.00
For Birth Certificates issued in CROATIA, SLOVAKIA and SLOVENIA:
- Report of Birth form - To be accomplished in four (4) original copies;
- Birth Certificate of the applicant - Original
- International Version and authenticated by the respective Ministry of Foreign Affairs (Croatia, Slovakia and Slovenia);
- Three (3) photocopies of all the pages;
- Marriage Contract or Report of Marriage of the parents of the applicant, if applicable - Original and three (3) Photocopies;
- Filipino Parent's Birth Certificate - Original and Photocopy;
- Parent's latest passports - Original;
- Three (3) photocopies of mother's passport (data page and last page)
- Three (3) photocopies of father's passport (data page and last page)
- Fee: € 25.00

For First-Time Electronic Passport Application (INFANTS/CHILDREN BORN IN AUSTRIA, CROATIA, SLOVAKIA AND SLOVENIA)

For Late Registration
Please submit a photocopy of parent's passports at the time of child's birth. If not applicable, any valid document as proof of parent's Philippine citizenship at the time of child's birth.
