1. Duly accomplished EMPLOYMENT CONTRACT;
2. Photocopy of Employer's LEGITIMATION CARD;
3. Photocopy of Employer's OFFICIAL PASSPORT (copy of data page and page with employer's signature);
4. Original Certification of Salary and Employment of Employer; and
5. Verification Fee:  € 35.00
