REQUIREMENTS FOR CONSULAR MORTUARY CERTIFICATE

For Caskets:

1. Leichenpass (Corpse Pass);
2. Embalsamierungsbescheinigung (Certificate of Embalming of deceased's person);
3. Auszug aus dem Sterbeeintrag (Extract from Death Certificate)/ Todesbescheinigung (Certificate of Death);
4. Photocopy of deceased's passport;
5. If Philippine national, photocopy of Report of Death;
6. Versandverfügung;
7. Photocopy of flight details;
8. Photocopy of passport of relative who will transport the casket;
9. Fee: € 25.00

For Urns:

1. Einäscherungsbestätigung (Certificate of Cremation);
2. Photocopy of Todesbestätigung//Sterbebuch (Certificate of Death);
3. Photocopy of deceased's passport;
4. If Philippine national, photocopy of Report of Death;
5. Photocopy of flight details;
6. Photocopy of passport of relative who will transport the urn;
7. Fee: € 25.00

Additional Requirements for caskets/urn from CROATIA, SLOVAKIA and SLOVENIA:

1. Documents must be authenticated by the respective Foreign Ministry;
2. English translation of the authenticated documents with one photocopy;
